Discount Program
Affordable healthcare for you and your family
East Valley Community Health Center offers a Sliding Fee Discount Program to help make eligible healthcare services more affordable.
You may qualify based on household income and family size — regardless of whether you have health insurance.
Patients whose household income is at or below 200% of the Federal Poverty Guidelines may qualify for reduced fees.
How to Apply
Ask our Front Office staff for a Sliding Fee Discount Program application. We can help you complete the Certification for Family Income and review available income documentation.
If you cannot provide traditional proof of income, ask about the self-attestation option.
No patient will be turned away due to inability to pay
