The Patient Portal is a safe, secure, and easy-to-use website and mobile app platform that offers you 24/7 access to your health information from the comfort and privacy of your own home or office.
Through the East Valley Patient Portal you can:
Complete necessary forms prior to visit.
Request to schedule an appointment.
Send a message to your Care Team.
View your lab results.
Request a refill on your medications.
How do I sign up?
Upon your first visit with East Valley, patients will receive an email and text message with a link to register for their Patient Portal.

The Patient Portal is for communicating NON-URGENT medical needs or concerns.
