East Valley Community Health Center has partnered with InteliChart to provide our patients with a new Patient Portal experience .

The Patient Portal is a safe, secure, and easy-to-use website and mobile app platform that offers you 24/7 access to your health information from the comfort and privacy of your own home or office.

Through the East Valley Patient Portal you can:

How do I sign up?

Users of East Valley's prior Patient Portal have been auto-enrolled in the new InteliChart Patient Portal and should have received an email between May 23 - 24, 2022 with information on how to activate their account. 

​

Individuals who were not registered with our previous Patient Portal will be auto-enrolled into our new InteliChart Patient Portal upon completion of their first visit after May 24th.

The Patient Portal is for communicating NON-URGENT medical needs or concerns.

  • If you have a true medical emergency, call 911 or call your local police or fire department.

  • If you have a need or concern requiring immediate attention from your provider or care team, please call the office directly.
