At East Valley, we believe that everyone has a right to cost-effective, quality health care. Our Certified Enrollment Counselors (CEC) provide individuals and families with information, a consultation, and support to ensure you can access the right health plan coverage for your individual needs.
Our CEC’s can assist you in applying for New Health Insurance Coverage or conduct a Benefits Review of your existing coverage. We work directly with Covered California, Medi-Cal, and Medicare, so you can be assured that you are accessing the best coverage options for you and your family. If you do not have insurance, East Valley can assist you with specific programs in accordance to their eligibility requirements.

NEW ENROLLMENT AND BENEFITS REVIEW APPOINTMENTS:
In order to offer you the best coverage at the lowest rates, please have the following documents available for your appointment:
SOCIAL SECURITY NUMBERS
Social Security numbers for U.S. citizens, or document information for immigrants with satisfactory status. Families that include immigrants can apply. Parents, you can apply for coverage for your child or children, even if you are not eligible for coverage.
EARNINGS
Gather all employer names, dates and income information for every working family member who lives with you. A “family” is defined as the person who files taxes as head of household and all the dependents claimed on that person's tax return.
INCOME VERIFICATION DOCUMENTS
Accepted documents related to income include a pay stub, Federal Tax Form 1040EZ, and an employer statement.
FEDERAL TAX INFORMATION
If a consumer doesn't file taxes, they can still qualify for free or low-cost insurance through Medi-Cal.
PROOF OF CITIZENSHIP OR LAWFUL PRESENCE. DOCUMENTS YOU CAN PROVIDE INCLUDE:

Document from a federally recognized Indian tribe that includes your name and the name of the federally recognized Indian tribe that issued the document, and shows your membership, enrollment, or affiliation with the tribe.
IF YOU DON'T HAVE ANY OF THE ABOVE DOCUMENTS, YOU MAY PROVIDE TWO OTHER FORMS OF IDENTIFICATION. OTHER DOCUMENTS INCLUDE:

UNDOCUMENTED MEMBERS OF THE COMMUNITY:
For those members of our community who are undocumented, you may still qualify for MHLA,
a public program available to provide medical coverage to those who do not otherwise qualify
for insurance. Our Certified Enrollment Counselors can assist you with securing coverage
according to your specific needs.
If you do not qualify for any insurance plan, you may qualify for MHLA,
a public program available to provide medical coverage to those who do not qualify for insurance.
